At The Outreach Center, we meet women everyday who are poor, lonely, addicted and struggling. They lack transportation, food, clothing, childcare, and are often mentally and spiritually drained. Help Wanted: Case Manager
We are looking for a dedicated and compassionate individual to fill the part -time position of Case Manager. Could that person be you?
This individual will work with the homeless to assist them in accessing mainstream benefits and permanent housing.
Intake and assessment of homeless, resource referral and advocacy; assistance with applying for mainstream benefits such as food stamps, disability assistance and Medicaid; and re-housing assistance, such as housing search, landlord and housing subsidy negotiations, locating and moving furniture into homes; and housing stability case management, including budgeting and follow ups, ensuring clients remain housed for at least six months.

An understanding of mental illness and substance abuse issues, demonstrates compassion and has an ability to communicate and develop trusting relationships with individuals coming from all walks of life, is fearless and willing to go above and beyond to assist the most challenging of clients within our community.
ADDITIONALLY THE CASE MANAGER WILL:
1. Create and maintain client files using ServicePoint™, a web-based homeless management information system database, as well as paper files, keeping accurate and up-to-date client information, service transactions, case notes and creating reports as needed
2. Travel and working with clients out of the office approximately 25% of time worked
This position requires that an individual have a high school diploma and a minimum of two years social service experience. Computer literacy, good communication and interpersonal skills, and patience and compassion for those in need are a must. Applicant must have a valid Louisiana driver’s license, acceptable driving record and reliable transportation. Prior Case Management / HPRP / ServicePoint experience a plus.
Bachelor’s degree preferred. Experience working with underserved or at risk populations , and/or experience providing housing services. A working knowledge of community resources and services is helpful.
Salary deemed upon experience.
